Early Childhood Approach – A guide for health professionals

Child development services (Tumbatin Developmental Services) Sydney Children’s Hospital, Randwick

The clinic provides an assessment and diagnosis for children with suspected delays or disorders in development. The clinic also provides counselling, advice, information and support for families and children who have a developmental delay/disorder.
Tumbatin Developmental Services Clinic works within a multidisciplinary team framework and includes paediatricians, social workers and clinical psychologists.

Transition Support- Childhood to Adult Services

Agency for Clinical innovation

    • Support plans for a transition from children’s to adult health services.

    • Find an adult healthcare service to suit you or your patient’s needs.

    • Empower the young person to adjust to new services, attend healthcare appointments and take on more responsibility for their own care.

    • Support families helping their young person transition.

    • Provide updates to health professionals on a young person’s progress.

Request a referral to the service by completing the referral form and submit it to your local Transition Care Coordinator.

Dentistry 

Sydney Dental Hospital

Special Care Dentistry is a referral centre concerned with the oral health care of adults with complex special needs. Their aim is to provide accessible dental services in an equitable and respectful manner to persons with disabilities. They offer a range of sensory tools and specialised equipment to assist during your visit. Treatment can be provided under local anaesthetic, Relative Analgesia, and General Anaesthetic.

Specialist Intellectual Disability Health Services

Specialist ID Health Teams

The Intellectual Disability Health Service offers:

Comprehensive health assessment and recommendations for a person with intellectual disability who has an unresolved complex or chronic health need that cannot currently be met by accessing usual care pathways, including:

  • Referrals for other health assessments and health services
  • Recommendations for support services
  • Dietary advice, advice on medication, physical activity, social activity
  • Advice on managing ongoing care

Capacity-building for health professionals, including:

  • Webinars, skills training and other education sessions
  • Joint consultation, case conferences and advice on assessment/care of people with intellectual disability.

CDS Complex Care Service

Service provides comprehensive healthcare assessments, healthcare plans, advice and referrals for people with intellectual disability. The service is based in Strathfield, and the clinic is run every Thursday.

Team includes:

The Disability Assessment & Rehabilitation Team for Young People

Multidisciplinary team (nursing, allied health and rehabilitation specialists).

The team provides a rehabilitation service for people with intellectual disabilities, multiple and/or complex disabilities. The service is provided to people with complex neurological and multiple lifelong disabilities aged 16-50 years and intellectual disability 16 years and over.

ID Mental Health Services

Statewide Intellectual Disability Mental Health Outreach Service (SIDMHOS)

Two Statewide Intellectual Disability Mental Health Hubs have been established to help people with intellectual or developmental disability access appropriate mental health care. One hub is for children and young people, and one hub is for adults. Both are funded by NSW Health.

The hubs offer:

  • a combination of face-to-face support and virtual care
  • specialised assessment, care planning, reporting and recommendations
  • education, training and support for mental health clinicians and disability workers providing care to people with intellectual disability.

Adult Hub

Referrals for case discussions, case reviews ad joint assessments require the referrer to complete the online referral form before an appointment is offered.
